An affidavit is evidence. It is a statement of fact made on oath by a named deponent, and once sworn it can be produced against them — a false statement in an affidavit is punishable, which is precisely what gives the document its weight. That is worth remembering when someone offers to 'just write anything' to get a form processed.
The form is settled. The deponent is identified with age, occupation and address; the facts are set out in numbered paragraphs in the first person; and a verification clause at the end separates what is true to personal knowledge from what is stated on information and belief. Getting that separation right is the difference between an affidavit that holds and one that is torn apart on cross-examination.
In Gujarat affidavits are made on stamp paper of the prescribed value, usually obtained through e-stamping, and then sworn before a notary or an authorised officer. The commonest uses we see in Bopal are name and date-of-birth corrections, address and income declarations, lost-document declarations for duplicates, gas and utility connections, passport and visa requirements, school and university submissions, and supporting statements filed in court proceedings.
The procedure, step by step

Establish what has to be proved
An affidavit is only as useful as its fit with whoever is going to read it. A bank correcting a name wants different content from a passport office recording a change of address. We start from the requirement — often a checklist or a rejection letter — and work back to the facts that need stating.

Draft in the deponent's own account
Facts go in numbered paragraphs, in plain language, in the order they happened. We avoid legalese here on purpose: an affidavit that reads as though the deponent could not have written it invites the question of whether they understood it. Only what the deponent can actually speak to goes in the body.

Attach and reference the proof
Where a fact rests on a document — a certificate, a receipt, an FIR copy for a lost original — the document is annexed and referred to by its annexure mark in the paragraph that relies on it. Unreferenced annexures are the usual reason a bundle comes back marked incomplete.

Stamp at the correct value
The stamp value depends on the type of affidavit and its purpose. Under-stamping is not a technicality that gets waived; it is a reason for refusal at the counter.

Swear it and take a spare
The deponent signs before the notary, the oath is administered, and the endorsement and register entry are made. We advise leaving with at least one additional sworn copy — institutions retain the original far more often than people expect, and a second visit for the same document is an avoidable irritation.

Where this goes wrong
Swearing to what you were told
Facts you know and facts you have heard belong in different categories, and the verification clause is where that distinction is recorded. Presenting hearsay as personal knowledge is the flaw that most often sinks an affidavit when it is actually tested.
Copying a format off the internet
Downloaded formats carry the wrong stamp value, the wrong verification, and frequently a jurisdiction that is not yours. They are refused often enough that redrafting properly is usually the faster route, not the slower one.
Leaving the annexures out
An affidavit that refers to a certificate without attaching it proves nothing about the certificate. Annex it, mark it, and refer to the mark.
One affidavit for several purposes
